Documenting and tracking plagiarism within the School

Whenever you have an instance of plagiarism in a class you are teaching (or in any other context in which you are involved), you are encouraged to document it via this form.

Data in the above form will NOT be added to the student's record, but is purely internal. This means that you can consult Student Services when you encounter an instance of plagiarism to find out if the student has a pattern of plagiarizing, which may inform the level of severity of your penalty.
